Mail
To receive a copy of this legal document, either
apply in person at the Clerk's Office or fill out the form below.
Mail the form along with a self-addressed, stamped envelope, to
the address posted on the form. The cost is $3.00 per copy.
Send a Cashier's Check or Money Order (please no cash and no personal
checks if you are not an Ohio resident) made out to "Shelby
County Probate Court." In order for us issue a certified
copy the license must have been issued by the Shelby County Probate
Court and must have been completed.
