Individual Forms For Adoption

Adoption Assessors

Listed below are the two local persons available to provide adoption assessments for step-parent, grandparent, relative, private, and birth adoptions.  These individuals have either been appointed by the Court directly or have been designated by the Court due to a contract with the Shelby County Department of Job and Family Services.  The local adoption assessors are:

Therefore, when an adoption is filed, on/after the date of this memo, the Court will no longer automatically select an individual to serve the role of assessor.  You will be responsible for contacting one of the above-mentioned individual assessors, or another qualified/certified assessor of your choosing, to make arrangements with them to provide the assessment.  That person’s name and information will need to be inserted on the form “Order to Assessor”.  If you choose an assessor other than Kathy Lindsey or Bridget Davis, please also provide the Court with the “Verification of Assessor Qualifications” that you can obtain directly from the assessor.

Here are the current fees charged by both designated and appointed adoption assessors:

    1. Grandparent and family adoption: $250 paid upfront directly to assessor by petitioner + $225 invoiced upon filing final report to Court, to be paid from deposit of petitioner; if deposit is  insufficient, petitioner shall immediately pay balance due to the Court and Court will then issue payment to the assessor.

Total payments to assessor = $475.00

    1. Birth adoption:  $650 paid upfront directly to assessor by petitioner + $225 invoiced upon filing final report to Court, to be paid from deposit of petitioner; if deposit is insufficient, petitioner shall immediately pay balance due to the Court and Court will then issue payment to the assessor. 

Total payments to assessor = $875.00

    1. Stepparent adoption:  $175.00 invoiced upon filing report to Court, to be paid from deposit of petitioner; if deposit is insufficient, petitioner shall immediately pay balance due to the Court and Court will then issue payment to the assessor. 

Total payments to assessor = $175.00

    1. Any additional fees shall be invoiced by the assessor and filed with the Court for approval and for payment from deposit; if deposit is insufficient, petitioner shall immediately pay balance due to the Court and Court will then issue payment to the assessor.
